TriBeCa Plastic Surgery is the office of Sydney R. Coleman MD, David L. Cangello MD, and Mary Lombardi PA.

Dr. Coleman is the world-renowned plastic surgeon who first observed the amazing regenerative ability of grafted fat and the stem cells associated with it.  Since 1987, he has developed pioneering procedures that utilize transplanted fat to correct liposuction deformities and enhance areas of the body such as breasts and buttocks. In the face, Dr. Coleman not only uses grafted fat to recontour and to restore youthful fullness, but also to harness the powerful rejuvenating and reparative abilities of fat and fat stem cells. To describe his technique of fat grafting, he trademarked the term LipoStructure® (alternatively referred to as Structural Fat Grafting, Coleman Fat Grafting or the Coleman Technique). Dr. Coleman applies these techniques not only for cosmetic applications, but also for reconstruction after radiation, cancer surgery, injury or for congenital malformations. He is actively involved in research to advance our knowledge of fat grafting and its potential for healing.

Dr. Cangello is a board-certified plastic surgeon highly acclaimed for the traditional cosmetic surgery procedures he performs, including facelifts, eyelid surgery, rhinoplasty, breast augmentation with silicone and saline implants, breast reduction and lift, liposuction and tummy tuck. He is an avid artist in his spare time, an interest and skill which has influenced his surgical practice. His precision and attention to aesthetic detail has earned him praise among both patients and colleagues.

Many patients would benefit from a combination of fat grafting and traditional procedures.   Dr. Coleman and Dr. Cangello team up for such procedures to optimize their results.

Dr. Coleman, Dr. Cangello and Ms. Lombardi are also skilled injectors, using neurotoxins (Botox, Dysport, Xeomin) and fillers (Restylane, Juvederm, Radiesse, Sculptra, etc.) to erase wrinkles, enhance facial contours and lift the cheeks.

Mary Lombardi, PA, a physician assistant with the practice, performs laser procedures (for dilated facial vessels, sun damage, hair removal, skin-tightening, etc.) and injectables, in addition to assisting with patient care before, during and after surgery.





One’s own body fat is the ideal filler for many reasons.  Firstly, it is autologous (one’s own) and therefore cannot cause an allergic reaction or “ rejection” as might happen with the use of artificial fillers or someone else’s fat.  For most of us, the fat is easily found in abdomen, hips or thighs, and we are happy to be rid of some of it!

It’s permanent!

There is every reason to believe that placement of autologous fat is permanent, unlike temporary fillers such as Restylane, Juvederm and Radiesse, which need to be repeated after a number of months. Dr. Coleman has been following his patients for many years, and photos show retention of grafted fat 10 years and more after placement.

It can be used to sculpt!

Unlike traditional silicone or saline breast implants, fat can used to sculpt the shape of the breasts to the patient’s individual needs or desires.  One breast is usually larger than the other, or shaped slightly differently.  Structural fat can be placed exactly where it is needed to make the breasts more symmetrical.  It can be used to fill in lumpectomy defects, scars and congenital malformations (tuberous breasts).  It can be used to enhance the cleavage, fill out empty sacs left by weight loss or breast-feeding, or to hide the edges of visible artificial implants.

The incisions are small!

The incisions used to harvest the fat, as well as those made to insert the cannulas for fat transfer, are small, approximately  1/4“ to 1/3”.  Incisions are usually hidden in natural body folds, the corner of the mouth, the eyebrow, underneath the breast, etc.

It feels natural!

Transplanted fat feels just like your normal body tissue.  It is impossible to tell transplanted fat from breast tissue visually or by touch.  Likewise, structural fat placed into liposuction defects or the face feels and looks natural.

You get two for one!

With Lipostructure, not only do you achieve correction or enhancement of the area of concern, but you have lost some unwanted fat from you donor sites.

Stem cells!

Studies show that the stem cells associated with transferred autologous fat can perform some remarkable functions.  Over time, transplanted fat has been shown to improve the quality and texture of the skin, including skin damaged by radiation, the sun and burns.  And there is evidence that the improvements continue for years after the original grafting. Patients also frequently report a return of sensation in tissues which were previously numb after mastectomy and other procedures.
